Staff at healthcare centers or hospitals assess the need for home nursing and send a service request through SAGA.
Heimastuðningur
Heimastuðningur er fyrir fólk sem á lögheimili í Reykjavík, er eldra en 18 ára, býr sjálfstætt og þarf stuðning vegna skertrar getu, fjölskylduaðstæðna, álags eða veikinda. Heimastuðningur er stuðningur við athafnir daglegs lífs, svo sem við að klæðast / hátta sig, sinna persónulegu hreinlæti, matast og nýta tækni við heimilishald og lyfjaeftirlit. Einnig félagslegur stuðningur, svo sem í gegnum skjáheimsóknir.
